The Tom Ritchey road bike featured the "death cleats" which locked your shoe in place by sliding the lever. Which is also how you enabled your shoe to come out again. Not that you are ever doing anything with your hands when you slow a bike down to the speeds where you might, y'know, need to get your foot out again. This design did not catch on.
